Register with GP

If you have recently moved to the area and wish to register with St. Clements Practice, please complete a patient registration form (GMS1) and also a patient questionnaire for adults or a patient questionaire for children for each member of your family. When you have completed these please bring the forms, with two proofs of residency (e.g. rent book, utilities bill, bank/building society statement etc), to the surgery during normal opening hours. If you have your Health Authority medical card please bring it with you but don't worry if you are unable to find it. You can register for your partner and for any children or other relatives who have moved with you; you do not each have to come to the surgery individually. You can obtain these forms at the surgery if necessary, or download copies from the above links.

After completion of your registration form, please contact the Surgery approximately one week later to confirm that registration has been completed.

We do ask if possible that new patients register with us as soon as they have moved, rather than waiting until they need to see a doctor, as this gives us time to get your details on to the computer and to request your notes from your previous surgery.
