Date Sharing and your choices

St Clements Partnership uses a record called a Summary Care record (SCR) which is an electronic record of important patient information, created from GP medical records. It can be seen and used by other authorised NHS staff in other areas of the health and care system involved in the patient's direct care. As a minimum, the record consists of:


· Current medication.


· Allergies and details of any previous reactions to medications.


· Name, address, date of birther and NHS number.


You have the choice to opt-out of this information being shared with other NHS professionals. If you wish to do so, please contact the surgery.
